During intestinal digestion, carbohydrates and protein are broken down into simple sugars and amino acids, respectively. The liver converts all of the sugars and some of the amino acids into glucose, simple sugar that is used for energy by every cell in the body. Glucose passes from the bloodstream into the cells with the help of insulin, a hormone produced by the pancreas (a pear-shaped organ located just below the stomach). By attaching to receptor sites on the surface membrane of a cell, insulin promotes the movement of glucose transport proteins from the interior of the cell to its surface, where they bind with glucose and carry it into the cell. In diabetes mellitus, several problems may interfere with this process: pancreatic insulin production may be partially or completely impaired, or body cells may become unable to respond to normal amounts of insulin efficiently.<br/><br/>These underlying problems in fact distinguish the two main types of the diabetes. In type 1 diabetes mellitus. Also known as juvenile diabetes, the pancreas produces little or no insulin. Type 1 diabetes develops suddenly and most commonly affects those under age 30; the average age onset is between 12 and 14. However, type 1 diabetes accounts for only about 5 percent of cases of diabetes mellitus. Continued fatigue<br/><br/>Type 1 diabetes is also known as insulin-dependent diabetes mellitus (IDDM). It&#8217;s less common in the US because the most severe cases usually develop within a few days or weeks. With IDDM, the lack of insulin stems from the destruction of beta cells. The symptoms associated with IDDM are very distinct:<br/><br/>Polydipsia : An extreme thirst that is due to frequent urination. The body is signaling for lost fluids to be replaced. Dehydration will occur if the condition is not caught immediately.<br/><br/>Polyuria : Urination that comes frequently and in large doses is a symptom of diabetes. The body rushes fluids through the kidney to dilute the unusually high levels of sugar in the urine.<br/><br/>Polyphagia: This is a feeling of unusual hunger. The body believes it is starving because glucose is not reaching its cells to provide the proper nutrients.<br/><br/>Extreme Weight Loss: There are many Type I diabetes patients that are at or below their ideal body weight. Insulin is a hormone that is produced by the pancreas to that helps the cells in our body to use the sugar in food. When it is unable to produce the hormone our bodies will have too much sugar in our cells and thus will have less energy to help us function on a daily basis. There are three main types of diabetes; type 1, type 2, and gestational.<br/><br/>Causes of Diabetes<br/><br/>The reasons behind these symptoms are the elevated glucose levels in the blood of the diabetic person and the insufficient glucose in the cells of the body where it is required. What causes these high glucose levels in the blood of type 1 diabetics is the lack of a hormone termed &#8220;insulin&#8221;. This insufficiency of insulin comes about when the body has assaulted its own beta cells responsible for producing insulin. The reason of the excessive level of glucose in the type 2 diabetics is the development of insulin resistance. Sometimes pregnant women also develop a temporary phase of diabetes termed &#8220;gestational diabetes&#8221; because of the pregnancy hormones.<br/><br/>Signs of Diabetes<br/><br/>Frequent Urination<br/><br/>This is the foremost warning sign of diabetes. When the body cannot metabolize glucose, a number of vital mechanisms can break down, sometimes with life-threatening consequences. &#8220;People don&#8217;t die directly from diabetes,&#8221; says Dr. Harvey Katzeff, &#8220;they die from complications. We do a good job of preventing complications, but a poor job of treating [them] once they occur.&#8221;<br/><br/>Diet and Exercise<br/><br/>Although Type 1 diabetes cannot be prevented, scientists are studying the genetic risk factors and are trying to find ways to suppress an immune attack. &#8220;With type 2, the picture is much brighter,&#8221; says the book Diabetes &#8211; Caring for Your Emotions as Well as Your Health. &#8220;Many of those who might be genetically susceptible avoid showing any sign of this disease simply by eating a balanced diet and exercising regularly, thereby staying physically fit and keeping their weight within normal limits.&#8221;<br/><br/>Underscoring the value of exercise, the Journal of the American Medical Association reported on a large study involving women. The study found that &#8220;a single bout of physical activity increases insulin-mediated glucose uptake [by the body's cells] for more than 24 hours.&#8221; Hence, the report concludes that &#8220;both walking and vigorous activity are associated with substantial reductions in risk of type 2 diabetes in women.&#8221; The researchers recommend at least 30 minutes of moderate physical activity on most if not all days of the week. This can include something as simple as walking, which, says the American Diabetes Association Complete Guide to Diabetes, &#8220;is probably the best, safest, and least expensive form of exercise.&#8221;<br/><br/>However, exercise by people with diabetes should be professionally supervised. One reason is that diabetes can damage the vascular system and nerves, thus affecting blood circulation and feeling. Hence, a simple scratch on the foot may go unnoticed, get infected, and turn into an ulcer &#8211; a serious condition that may lead to amputation if not treated immediately.<br/><br/>Insulin Therapy<br/><br/>Many with diabetes must supplement their diet and exercise program with daily testing of glucose levels along with multiple insulin injections. As a result of improved health through diet and a good routine of exercise, some with Type 2 diabetes have been able, at least for a time, to discontinue insulin therapy. Karen, who has Type 1 diabetes, found that exercise increases the efficiency of the insulin she injects. As a result, she has been able to cut her daily insulin requirements by 20 percent.<br/><br/>If insulin is needed, however, there is no reason for the sufferer to feel discouraged. &#8220;Going on insulin does not imply failure on your part,&#8221; says Mary Ann, a registered nurse who cares for a number of diabetes patients. &#8220;Whatever form of diabetes you have, if you carefully control your blood sugar, you will minimize other health problems later.&#8221; In fact, a recent study revealed that people with Type 1 who kept strict control of their blood-sugar levels &#8220;had drastic reductions in the occurrence of diabetic eye, kidney, and nerve diseases.&#8221; The risk of eye disease (retinopathy), for instance, was cut by 76 percent! Those with Type 2 who keep tight control of their blood-sugar levels enjoy similar benefits.<br/><br/>To make insulin therapy easier and less traumatic, syringes and insulin pens &#8211; the most common tools used &#8211; have microfine needles that give minimal discomfort. &#8220;The first shot is usually the worst,&#8221; says Mary Ann. &#8220;After that, most patients say they hardly feel a thing.&#8221; Other methods of injection include automatic injectors that shoot a needle into the skin painlessly, jet injectors that literally fire insulin through the skin by means of a fine jet blast, and infusers that employ a catheter that stays in place for two or three days. About the size of a pocket pager, the insulin pump has gained popularity in recent years. This programmable device dispenses insulin through a catheter at a steady rate according to the body&#8217;s daily needs, making insulin administration more precise and convenient.<br/><br/>Keep Learning<br/><br/>All told, there is no blanket therapy for diabetes. Genetic makeup may also be implicated, for Type 1 diabetes often runs in families, and it is most common among Caucasians.<br/><br/>With Type 2 diabetes, the genetic factor is even stronger but with a greater occurrence among non-Caucasians. Australian Aborigines and Native Americans are among the most affected, the latter having the highest rate of Type 2 diabetes in the world. How do I begin an exercise program?</strong><br/><br/><strong>A.</strong> Schedule a physical exam with your physician before beginning your exercise activities. This is especially important if you have not been active recently. An EKG or ECG test (electrocardiogram) may be recommended by your doctor along with a graded exercise test to evaluate how exercise may effect your heart. Ask your doctor what types of exercise are best for you.<br/><br/>For any recommended exercise program, remember to begin slowly and safely, and gradually increase your physical activity level over time.<br/><br/><strong>Q. What is an exercise goal?</strong><br/><br/><strong>A.</strong> Remember to discuss with your doctor how often to exercise, and what level of intensity at which you shoudl perform. Begin any exercise program with a slow, steady pace. Try to be active for at least 20 minutes in the beginning. You can steadily increase the time period for exercise over an eight (8) week period.<br/><br/>Research has proven that weight lifting (anaerobic exercise) may benefit people with diabetes. This type of exercise is known as &#8220;resistance training.&#8221; Lifting heavy weights may not be helpful for persons with certain diabetes complications, so always comply with your physician&#8217;s recommendations.<br/><br/><strong>Q. What are the different types of exercise?</strong><br/><br/><strong>A.</strong> There are two main types of exercise, anaerobic and aerobic exercise. Anaerobic exercise involves lifting weights or using exercise bands. Activity usually lasts for a shorter duration and involves intense activity done in short bursts. Anaerobic exercise is commonly known as &#8220;resistance training&#8221; and the goal is to build muscle and increase strength.<br/><br/>Aerobic exercise works the large muscle groups in the arms and legs. Examples of this include: jogging, brisk walking, swimming, biking, dancing, biking and rowing. Aerobic exercise gradually increases the heart and breathing rates. Aerobic exercise is usually recommended for most diabetic patients because of the proven benefits to the cardiovascular system (heart, lungs, and blood vessels).<br/><br/><strong>Q. What if I have had a sedentary lifestyle?</strong><br/><br/><strong>A.</strong> You will improve your overall health by increasing your activity level. Take advantage of any opportunity to bend and stretch as much as possible during the day.<br/><br/>To make your day more active, here are some simple tips to get ready, get set, and get moving:<br/><br/> Parking your car further away from the store when you go shopping Use stairs rather than elevators whenever possible Communicate with neighbors or co-workers by stopping by in person, rather than using the phone to call Stay active at home with light gardening and housework chores <br/><br/><strong>Q. What are the strategies for exercising success?</strong><br/><br/><strong>A.</strong> Make exercise part of your daily schedule &#8211; even small increases in physical activity will be beneficial to your health. Being active simply means moving more!<br/><br/>All physical activities should begin with a warm-up, and then progress to the chosen exercise. After exercising; remember to have a cool-down period. You will see the most progress with your exercise plan when you are consistent with a specific type of activity. For example, if you chose to walk, try to find the most convenient time of day for walking, and then stick to it.<br/><br/>Your exercise program should always include these three (3) essential elements:<br/><br/>Warm-up: Approximately 5-10 minutes of stretching and bending. This will prevent injuries to muscles and joints.<br/><br/>Activity: 20 minutes (or more) of sustained physical activity. This includes walking, biking, aerobics, swimming, or other sports, such as tennis or racquetball. Cool-Down: Gradually reduce your activity, and then stretch for at least 5 minutes.<br/><br/><strong>Q. Are there any exercise precautions or other guidelines?</strong><br/><br/><strong>A.</strong> Remember, exercise will lower your blood glucose level. If you feel symptoms of hypoglycemia (low blood sugar), take glucose tablets or gel to treat low blood sugar. Avoid very vigorous activity when the insulin activity is peaking (insulin is working at the hardest level). If you plan to exercise at moderate or high intensity, check your blood sugar before beginning your exercise. If the blood glucose level is below 100 mg/dl, eat a light snack with a carbohydrate source before exercising. Remember, low blood sugar can occur shortly after exercise or up to 24 hours after physical activity, so monitor blood glucose levels regularly.<br/><br/>When purchasing shoes for exercise, protect your feet with properly fitting shoes and socks. Plan to purchase your shoes at the end of the day when your feet are larger. Avoid open-heeled or open-toed shoes.<br/><br/>Shoe-Fit guidelines: the in-step of the shoe should conform to your the size and the shape of your foot. Your toes should have some &#8220;wiggle-room&#8221; &#8211; the shoes should securely fit at the heel to prevent slipping.<br/><br/>Sock Suggestions: White socks should be used, and they should be soft and padded with no seams.<br/><br/>Diabetic patients need adjustments in insulin dosing or eating schedule. Food intake may need adjusting to prevent low blood sugar when exercising. For example, if you will be bike riding, do not<br/><br/>inject insulin into your leg. Taking control of diabetes will be extremely challenging.<br/><br/>However, living with the disease does not mean that your life is over. As a matter-of-fact, taking good care of yourself and taking care of your diabetes will help you feel better.<br/><br/>Although it is not really known, even to modern medicine, the ideal cause for diabetes, it is clear that there are means to control progression. In order to take control of the disease, you must first learn about it.<br/><br/>The disease is the make-up of a metabolic syndrome that suggests that the body utilizes digested food to promote growth and energy. Basically, the main source of energy is food. The digested food is broken down into glucose. After which, it is absorbed by the bloodstream to be distributed to the cells of the body.<br/><br/>The glucose requires the assistance of the hormone called insulin to travel to the cells. Meaning, the insulin, which is produced by the pancreas (gland behind the stomach) is the key that allows glucose entry to cells. Ironically, the insulin also plays significant role in what causes diabetes.
